You may want to try FAR, a shareware program available from
http://helpware.net/FAR/index.html. Once you've installed this
program, you can quickly build a .chm file by opening Windows
Explorer, right-clicking the top-level directory that contains your
source files, and selecting "Compile to HTML Help with FAR" from the
context menu. Then you can use FAR to re-arrange the TOC, add further
index entries, customise the appearance of the help window, and so on.

FAR's requirements include HTML Help Workshop, which you can download
from:

http://msdn.microsoft.com/library/en-us/htmlhelp/html/hwMicrosoftHTMLHelpDownloads.asp
